Patient visits are by appointment only. Appointments may be made by calling 336-940-2407 between the hours of 8:00 AM and 5:00 PM Monday thru Friday. To ensure that we allow adequate time to address your needs, please specify whether or not you wish to make an appointment for a complete skin exam or for evaluation of a specific issue.

We no longer require a referral for new patients. However, it is the responsibility of the patient to get a referral if required by their insurance company. If a referral is required by an insurance company but was not obtained, the patient is responsible for all of the charges incurred during the visit.


We have reserved your appointment especially for you! We appreciate a minimum of 24 hours notice for any cancellations, so that we may offer that appointment to another patient. We reserve the right to charge a $50 no-show fee for any appointments missed without cancellation.

New Patients
As a new patient to Davie Dermatology & The Med Spa, you will be asked to fill out paperwork prior to seeing the physician or physician’s assistant. Please arrive 15 minutes before your scheduled appointment time so that this paperwork may be completed. If you prefer to fill your forms out at home and bring them with you to your appointment, you may download and print forms HERE.
What to Bring

We ask that EVERY patient bring their insurance card with them to EVERY visit. This assures that we file your visit correctly with your insurance company. If we have your correct insurance information, we will file your visit and any medically necessary procedures with your insurance company. We do not file cosmetic or medically unnecessary procedures or treatments with your insurance company, and require full payment for those at the time of service.
