We now include paperwork with our appointment reminder messages. It may take a little longer to fill out the forms the first time. You can access the forms from the appointment reminder email or text message that we send you 3 to 5 days before your visit. For future appointments, you can simply confirm that your information is still correct and sign any new or expired consent forms via the reminder message. The forms below are available if needed, but you don't need to fill them out for upcoming visits.
